Patient Folder

All patients receive a white Chartwell folder with their first delivery. This folder contains very important information concerning therapy such as instructions for administering therapy as well as medication information. Please be sure to read through the documents in the folder and contact us with any questions.

• Patient Consent and Certification
  By signing this form you are giving your permission for Chartwell to provide treatment to you. This form also gives permission for Chartwell to obtain necessary information from your physician, hospital and nursing agency, which may be required to provide care. In addition, by signing this form you authorize your insurance company to provide payment directly to Chartwell for services provided.

We feel continuity of care is crucial in order to achieve a positive therapy outcome. Chartwell utilizes a team approach to provide individualized care and clinical monitoring to all patients.

Chartwell has four Patient Care Management Teams, which consist of highly skilled infusion nurses, pharmacists, reimbursement specialists, customer service representatives and support personnel.
