Initial Referral to Home Infusion

Home infusion originates with orders and a prescription from a licensed physician. A referral is then made to the home infusion service either directly from the physician, from a hospital discharge planner, case manager, or the patient’s insurer. Complete patient and prescription information must be gathered at the time of initial referral.

In many cases, it is expected that treatment will be initiated within a matter of hours—either because of dosing requirements or because the patient is ready to be discharged from the hospital. The referral process must be thorough, accurate, and efficient.
