Admission Coordination
After intake, clinical staff work together to coordinate the
initial visit and develop the plan of care. Clinical team members
within the organization typically include clinical pharmacists
and nurses specializing in infusion therapy. Clinical staff must
also coordinate and communicate with other care team members outside
of the organization, such as key hospital staff, the physician,
the patient’s insurer, and the patient’s family.
In some cases, patients may be receiving other home health care
or supportive services while receiving infusion therapy. These
must be coordinated to provide optimal patient care, prevent duplication,
and ensure the optimal use of resources.
