Chartwell is recognized for it's clinical expertise
and dedicated staff. Utilizing a Multidisciplinary approach, all
patients are provided with comprehensive services including 24 hour
/ 7 day per week Emergency Support as
well as ongoing Inventory Management and Delivery
Coordination. Specialized services within each department are
listed below.
Emergency Support - After
business hours pharmacy, nursing and delivery services are available
through an answering service for consultation by calling 1-800-755-4704.
Inventory Management - Customer
Service Representatives routinely contact patients prior to each
delivery to review supply and drug quantities / needs.
Delivery Coordination -
Customer Service Representatives coordinate each delivery with
the patient to ensure timely receipt of products.
Pharmacy Services
Pharmacokinetic monitoring
including evaluation of drug and dosing appropriate to diagnosis/therapy,
monitoring of medication profile, potential drug/food interactions,
allergies and sensitivities, first dosing in the home and anaphylaxis
precautions as well as ongoing clinical monitoring. Prescriptions
are processed electronically with specially developed software
to minimize medication error potential.
Nursing Services
Specially trained infusion
nurses provide individualized patient education, coordination of
care including assistance with travel coordination, ongoing clinical
assessment and monitoring. Nurses are Certified to administer chemotherapy
and insert PIC lines.
Pediatric Services
Specialized pediatric team
consists of pharmacists and nurses with extensive pediatric infusion
experience. The pediatric team cares for all children from birth
to age 18 to ensure developmental milestones as well as medical
needs are being met.
Dietary Services
Specialized enteral therapy
team provides nutritional assessments by Registered Dietitian utilizing
a Dietitian-developed enteral acuity model. Ongoing evaluation
of patient response and tolerance to therapy with recommendations
made to physicians as needed.
Reimbursement Services
Our team of Insurance
Verification Specialists assists with identification and verification
of insurance benefits/policies. Each patient and referral source
is contacted prior to initiation of service to discuss insurance
benefits and coverage. Patients are notified if they have any financial
responsibility.
Billing Services
Our billing team ensures accurate
claims to third party payers and works with patients and their
families to address out-of-pocket responsibilities.
